Dr. Herbst reserves up to two hours for each patient's treatment. Please arrive early for your first appointment to complete your registration. Please show us the courtesy to be on time to all of your appointments. Arriving more than 15 minutes late or failing to give 48 hour notice for cancellations and changes will incur a cancellation fee of $100. We appreciate your cooperation and understanding. Insurance Policy:We make every effort to keep the cost down of dental care. You can help us by keeping scheduled appointments, and by paying for treatment at the time of service. Dental insurance is a valuable asset and assists patients in paying for a portion of restoring and maintaining good dental health. We do, as a courtesy to you, file dental claims. However, due to constantly changing insurance benefits and deductibles, we are only able to estimate your insurance balance. If your insurance company pays more or less, we adjust your account accordingly. The patient is responsible for making us aware of any change in their insurance and for the balance not paid by insurance. We do ask that your co-payment be paid at the time of service. A co-payment of 30-50% is expected. We do participate with many insurance companies. If you have any questions, please feel free to call the office and we will be happy to assist you. Please contact our office with any questions. Patients without Dental Insurance Coverage:If you do not have dental insurance, please call our office to discuss special arrangements.
